I also have a sample data spreadsheet with the data on that I need mail merging into the document, and then the PDF form being emailed out to each person. The idea was to convert the PDF to word, then from word mail merge this out (so each person got a PDF with their own details on), each month the data will change, so I need someone to set this up and I can do mail merge each month going forward, the same data spreadsheet details which sections of the form I want to mail merge information into. Also some of the boxes require ticks in them like the title. So if the title is Mr/ Ms/ Mrs it puts a tick in that box, if it is something different then the actual title goes into the boxes to the right. I have attached the PDF and also a sample spreadsheet - I tried converting to word and doing a mail merge from there, but I was struggling to get the information I needed into the relevant boxes Please have a look at the form and see if this can be done before bidding as it isn't a quick 5 minute job to do this! Naturally there will be different levels of query: basic, important, deep restricting how much information is made available based on the query level and the person being queried must approve first. Either by text or phone or via app Secondly, interface to handle this electronically. The interface that queries or loads information on the database must be such that it can operate from phones by directly going to the website and specifically to the information held about the party who’s card was scanned by barcode or near field technology. It must also work seamlessly in places where the Internet is not of best quality. But without approval (one time code sent via text and email ) the query will not be successful
