2) Have you made changes/fixes to the site to address the de-indexing?
3) What is required now?

Once you've provided full details, you'll get proposals from professional freelancers. Without full details, you'll receive only proposals for amatuers, chancers and scammers.

I'll leave that one up to you, but I'd strongly advise you to be thorough and provide full details, for your own safety.
