Simple Taxi Request mobile site (can Be viewed in desktop browsing)

Description of requirements/features: I need a simple Web site that can be visited either from computer or mobile to do the following :

1- Allow the customer to request a taxi (From , to).
2- Add all Customer Information related to trip.
3- Ability to get current location from mobile.
4-Ability to choose location from map (for both from and to)
5-find a way to update the customer with (taxi location) and allow customer to see it.
6- If possible notification to customer.

Here how the system will work :

The customer will request a taxi, We should receive the request Immediately (instantly) by eaither Email or SMS or any other fast notification.With IP address

We will analysis the request and through our taxi network we will find the most suitable taxi.
We will send the taxi details and cost to the customer by SMS.
-Optional The customer should be able to see where is the taxi approximately now.
-The Customer should be able to register for future.
-Only IPs from Oman should be accepted.

The Form Requirements :
-Name
-From (Can add current location , choose from map , Drop down menu , OR insert new location).
-To (Can add location , choose from map , Drop down menu , OR insert new location).
-Mobile (Should only accept 8 digits starting with 9) (symbol +968) before the number should be there.
-Email
-Number of passengers.
-Number of Luggage.
-Any special requirements (notes).
-recurring Event (just notification of recurring time and day a text box would be enough as a start).

all fields should accept arabic and english
Visual requirements :
The form should be attractive using our logo , Design of the form can be done by you or we can find another designer for the form (please specify if you can do an attractive design or you prefer to get a designer).

Hosting , SEO should be done by candidate.
Source code should be in our ownership
CMS and Admin requirements: Currently no need but in future we will need to manage accounts , and customer will be able to register and see saved work.
