Simple javascript form to send data to an API

I need a very simple javascript page to be built which has a form into which a user can input some information which will then be placed into a JSON to be sent to an API.

The only bit that makes it slightly more complex than just a simple form is that there is one field which should create a list of objects with multiple parameters.

So if the form has:
- title
- description
- questions

the questions input should allow the user to input multiple questions (and remove/re-order them). For each question a user can provide several parameters
e.g
- type
- question_title
- required
etc.
