Finish a PHP form process on WooCommerce

Description of requirements/features: We have a custom PHP form on our Ecommerce site that aids the checkout process to create a custom product.

The stages are extremly similar to the refrenced site.

We have had a developer working on it but he is unable to finish and we need the form completed ASAP.

for the most part the form seem's to be complete what we need is for someone who really knows there stuff to swoop in and finish it off.

This is a UK preference job and I will only accept proposal's that start with **ME**
