Long PDF form into easy step by step online form

The project is to turn a long and confusing PDF customer registration form into a step by step process that can be emailed to customers. This might be a link to a webpage hosted on our site. Customers currently receive an editable PDF form and often do not have the correct equipment or think it is to complex to complete.
We would like them to go through the form step by step with a more visually appealing format that is less daunting. Adding "help" for certain questions will be important, mandatory or optional questions, specifying format of questions such as dates and a counter to show how much of the form is completed.
At the end the form requires a date stamp and E-Signature to confirm accuracy of the data supplied.
Once completed the data is to be formatted back to PDF for the "Office Use Only" sections to be completed by our admin team.
The signed PDF version is sent to the customer automatically as well as ourselves once they have signed and submitted it.
