Simple data collection website

Hi, I need a simple page to collect data and populate a mysql database or similar. About twenty fields, mainly text. Will also need a little help with accessing the data generated. Needs to work on desktop and mobile but doesn't need to be fancy or even that pretty.

If the project grows, there will be more work, if not, not. No particular budget, but will pay what's necessary. Need someone with solid skills to run something up quickly for me.
