Looking for an Order Form for wordpress website

Hi i m looking for someone to build me an Order Form for a website i m working on.
the Order form inside is a box with 6 small boxes with a Plus Button.
what i would like it to do when the Customer clicks on one of the Gray Plus button it opens a product list where they can select what they want to buy.ones it is selected it goes into the box showing in the picture.to the right the description with price.total on the bottom and an area where they can pay.to the left where i can put the company logo company info.ones order is submitted owner gets an email an order came in.
