Custom Wordpress Plugin

I require a custom plugin built as follows:

Admin area
- Enter content in five fields (3x text, 1x textarea, 1x image file)
- This information is stored in the db
- The admin can go to this page again and change the information at any time

Frontend
- Pull out the five fields onto the page
- A dropdown field (can be hard coded)
- Additionally, show a URL on the page that is built up of dropdown field + text field 1 + text field 2 + text field 3

Wireframes provided here. I can do the styling if necessary, the important part is the PHP.
