Convert print book (pdf) into eBook (ePUB)

My print book has already been designed, and I have the pdf file. I need to convert this into an ePUB file to obtain an eBook. The file for the print book has a table of contents, footnotes, and other features that would need to be changed to conform to eBook standards. The book has under 200 pages and contains approximately 30 black and white photographs. The eBook will be published via Lulu.com and needs to comply with all the requirements for Global Distribution (type in 'Lulu Mandatory Ebook Distribution Requirements' on google, and familiarise yourself with it).
