To Create a data base

We are restaurant and would like to keep a database of when last a customers.

The reason for this is to see when a customer has/hasn't ordered for a period , so we can send them an offer

We intend to add name, address, phone no. & date of order

We would like to see some sort of chart of frequency of customer order.

Hower when we enter the information we do not want double of names

It must be simple to use and written instructions of how to use database
