I need aPDF document made with form fields

I have a business where I train security and law enforcement people. We train them in many ways. One thing we want to work on is their ability to complete a concise "incident report". I have a document I need done professionally and I need it done so that the information can be typed in and saved. But, I need to have a template made so we can use it over and over. We would need both the
questions and the answer sections set up for easy changes to be made by my staff and I. We may even want multiple forms made. We need them to be different so that the trainees have to adapt to the different type of reports needed. I have a budget of $50-$100. We have a need for it now!!
