Summary of the system
Form digitization and workflow automation high-level requirements
• Digitization of the forms used at the plazas – sample forms included. The forms are primarily alpha character based with minimal or calculations required
• Ability to digitally capture the daily operations checklists
• Ability to capture images as supporting document, with geo-tagging capabilities of the place where the form or image is captured
• Allow form capturer to digitally accept terms and conditions and sign off the checklist sheet
• Submit the captured form digitally – in instances where dependent workflows exist, that the interface will allow for relevant data to trigger the subsequent processes
• Central repository to access the captured checklist and its related data electronically
Expected deliverables
1. Forms design
1.1 Each line on the forms would be entered in the designs
1.2 Geo-tagging capabilities will be on each form that every submitted form can show the location where the form was submitted
1.3 Signature fields will be added to each form
1.4 Each form will have a PDF saved to a specific email/repository system. This will be the master form and will be updated
1.5 Each form PDF will be designed, aligning the placeholders on the PDF for each field on the
form
1.6 Setting the workflows for destinations where the pdf should be send to after each submission
2. Forms format
2.1 The submitted forms should be accessible either in PDF, CSV or XML format
2.2 Submitted forms should allow for integration to other organisation databases
2.3 User Interface – define how the user will be interacting with the solution
3. Project management and implementation

Hi Fungai
We have understood that you use paper forms at the moment. So when digitizing this form through scan or image capture of the paper form, would you like the content, according to the corresponding row and column to be entered into the database in image form, or should the handwritten text be converted to digital, type written form? Of course the latter is a much better option but has technical complexity and the handwriting should be legible enough for the system to recognize. Please do let me know your preference. -
What happens if no connectivity is available in the area where the forms are being signed?
