What the Freelancer needs to start the work

I will need your design in html with css and js .
Number of pages are limited, user registration is not in the hourly .
Site can have 1-2 simple form and a newsletter form.
Commonly used plugin are used for the form. If there are complex calculation on pages, please notify as charges may increase . (Depending on other works)
