English to Italian Questionnaire Translation

Hello,

We are a market research consultancy currently planning a quick online survey on a concept car displayed at the Geneva Motorshow in March 2016. We need the questionnaire to be available in Italian and therefore require:
- Translation of the questionnaire (English to Italian) - 1200 to 1500 words
- Online proof reading of the questionnaire (once programmed) and input into potential corrections
