The decision will be made at 16:00 GMT 02-Feb-2015 or sooner for the right price/time proposal.

Your proposal MUST:
1) indicate when you can have the job completed
2) include how much you will charge - itemised where directed
3) provide a link to something very similar you have completed previously
4) include a list of prerequisites you require to get the job done

Your proposal MUST NOT:
1) contain any self promotion - I can read your profile and feedback for that
2) offer any other services other than the job in hand

If you cant get this right, I can only conclude you are unlikely to be able to interpret the very much more specific details of the brief correctly.

I have a small website built using wordpress Theme: Convention by Fimply
I have a form on the site that I have built (crudely) in PHP that allows the user to input data and then submit it (adds a row to a MySQL table) - it sits outside the WP environment and therefore looks bad.

The option to build the form in WordPress seems cumbersome and so I would rather just connect this page to the WP CSS and get it styled up to match the rest of the site.

The form consists mainly of text boxes but there are some dropdown lists.

It needs to look good, be easy for users to input, work well on mobile phones. it also validates each field.

The attached file indicates the name of the website and is a screen print of the form (you can of course browse to the live version).

Please note there is also a CAPCHA - will need something like this as part of the solution.

There are some fields on the form that I would like to be defaults from other entry fields - if you can do that.

There are collections of fields that i would like to be populated by pressing a [copy] button - if you can do that.

There are some rows of fields that i would like to only appear (as repeats of the first row) when the user clicks [Add more]

I would be receptive to any other suggestions as to user experience or styling enhancements.

Note: expected budget is not a fixed price - you to provide your cost
