Intro / Outro video - graphic for youtube channel

Looking for an intro video made for a youtube channel which will focus on videos of crazy car drivers/and near bike accident misses and any other 'random' moments caught on camera.

I would also like to have sound effects (quotes) to make the videos more comical and engaging and to have the channel name/logo on the screen in one of the corners (much like royal Jordanians'). I particularly like Moto madness intro voice over 'oh what a day, what a lovely day' which is taken from Mad Max film.

I plan to use adobe premiere elements to edit the and add the actual video footage, so the intro piece produced by you will need to work in this software or similar etc, so that I can use and re-use again and again on my videos.

in terms of music I haven't decided on this, but it would need to be in theme with the channel and what material I will be displaying i.e crazy 'omg moments' caught on camera/ car near misses/ angry bikers etc

Timescales, I would like to think it can be made and finalised within 5 days.
