Woo-commerce custom product option

We are looking to add the ability for people to customise some of the products on our website, this would include them when buying a product having the option to have the product custom engraved.
If selecting this option the first product would be £10 for example any further products would be an extra £5 per product for the same engraving.

There would need to be an upload box and a description box so the customers can explain what it is they are after.
There would also need to be on some of the product types the option as to which part is getting engraved with different parts costing different amounts.

This would need enabaling on a product by product level so that we can highly restrict which products customers can select the option for.
We also would need a standalone product/form if someone wants to send us there products to have engraved instead of buying from us and having it done at the time of purchase.
