Webpage to display data from mysql database

Hi, I am looking for someone to make a webpage that will have a few dropdown boxes to select date range, and a couple of other criterion and retrieve the relevant information from the database.
It doesn't have to look really stylish but would like it to be neat and easy to view.

I already have the database set up and connection is good. I use a simple php script to handle the requests.
I have already made an android app that inserts data, that is working fine.

Essentially i would like a table displayed with about 5 filter options to display relevant data.
