We need a CSS/Java/HTML popup to collect client information

Description of requirements/features: We are looking for someone to create a customer information and contact details collection pop up for our website. The customer will answer a series of questions, then enter their contact details. When submit is clicked at the end, an email will be sent to us containing the answers to the questions and the customer's contact details.

This website shows the style of popup we would like you to reproduce: https://www.bark.com/en/gb/telephone-systems/?popup=true.

One change to this style that we would like is to move the 'close cross' from the top right hand corner of the screen to the top right hand corner of the dialog box (see attached picture). Obviously, we will provide the actual text and questions we would like to appear for our own website.

We would like this popup to appear on 8 - 10 pages of our website, including the homepage and possibly more in the future. Clear code relating to the HTML, CSS and Java assets will be necessary so we can copy them over to future pages.

We will likely require further web development work in the future and are looking for high quality results at a reasonable price, with a view to building a longer-term working relationship.
