Updating subscription pages Shopify already using Bold Recurring

Looking for help with updating our subscription pages. We already use Bold Recurring so need someone with a good understanding of this app. We have created a visual to give an idea of what we are after - trying to achieve a very simply ordering process. The look to be in keeping with our Branding.
