I need a web developer to code a 4 - 5 step contact form.

Description of requirements/features: I'm creating a Wordpress website that sells customized cricket stumps.

I need a contact form developed (4 - 5 steps) that allows website visitors to enter their contact details and pick which colours to use when customizing the stumps and add text to the stumps using the contact forms.

Need to be very similar to this and have all the same information in the form along with the stumps picture beside the form that changes colour when details such as colours and text are added into the form.
Extra notes: I would like the the code for the form so I can just copy and paste it into the Wordpress page.
