I need a web application for the input of timesheet hours worked by emloyees.

General information for the website: We are Recruitment agency and provide temporary staff to Care homes
Num. of web pages/modules: maybe three
Description of requirements/features: The web application will be used for the input of hours by office staff against a candidate name and the place he/she has worked.
1. Login interface to show who has done the input of the timesheets.
2. Drop down menu for selecting candidates and place of work, which the administrator should be able to create/populate and delete as need be.
The place of work will have different rates of pay, for example, Mon might be 20p/hr and Saturday might be 25 p/hr.
3. Staff must be able to select a date and if the employee worked a day or night. Night and day rate are different for each place of work.
4. The web application must be able to calculate a total for each candidate against the hours worked and the rate of pay for each place of work.
5. A row with a heading for each day of the week must be available for the staff to input the actual hours worked.
5. A staff member must be able to download a pdf and CSV that can be customised to show different fields eg place of work, the name of the employee, dates worked and total hours worked and the total cost for that week.
6. A database must be somehow available for searching entries using week ending dates.
