I need a multi-page form created.

I need a form created for my physiotherapy business which asks the user various questions before sending them to a URL to book their appointment there is an example of exactly what I'm looking for available if you DM me and you can see what we're looking for I have all the mapping everything done I just need some help building forms you can use either gravity forms or WP forms which is what it's currently built in I need this done in the next 24 hours however I'm happy to input once the form is set up
