I need a CSS style sheet fixed that works with a basic Javascript form

General information for the website: This is a basic Javascript form that allows a user to enter inputs, hit submit, and draft a form letter that they can print out
Description of requirements/features: I have three files, an html file, a javascript file, and a css style sheet. They work together to allow a user to enter some inputs, hit submit, and draft a basic form letter that they can print.

The input menu has three problems, which I think are probably a CSS problem:
1. I need to eliminate the unnecessary white space (width) in the drop down menu on the page.
2. I need to make the text wrap in an explanatory paragraph on the user input menu.
3. I need to make the input fields line up.

It would be great if the CSS also allowed the menu to be responsive to scale to different devices, but that's not necessary.
