I need a basic wordpress Landing Page

Hi,

Very important: (This project if accepted must be completed by Friday 21st Jan, 13:00pm gmt.

1.Design landing page (reference will be provided)
2.On this landing page there will be a (box) to enter a registration number.
3.Once the registration has been entered and the user clicks on (Go), they are then taken to a form with approx 5 fields to fill in.
4.When the form is filled in, the information is then sent via email and stored within wordpress.

(Only other additional pages will be the following:

Terms and conditions
Privacy Policy
Cookie and data notice.

(That's it) :)
