Disclaimer: I'm not sure this is the right category, but I had to choose, so I'll start here =)
I have a kind of personality test which creates a profile based on nine different categories in three larger groups. There are 72 questions, where each one presents a pair of statements (each corresponding to one of the nine categories), where the subject chooses one over the other.
So ideally, the options would read 1a/b, 2a/b etc, but each a and b option should be assigned a value based on which category and group they belong to (known in the back-end but not to the subject).
Up to now I've used a PDF form (enclosed), but what I'd like is a web form where the subject only sees the options "a) XXX", "b) XXX", "both high" and "both low" for each question (rather than seeing where in the categories a and b fits, as is the case on my PDF). After their completion, they should click "submit", whereupon I would receive an email with their resulting profile (how many score points they gathered for each category as well as each group in total) and also containing their actual test answers for reference.
The "both high" and "both low" options for each question (i e each pair of statements) should be graphically different from the a/b choices. Furthermore, the subject should only be allowed to choose one of these instead of one of a/b on a set maximum number of questions — so when someone has gone with high/low for, say, five questions in total, this option should be grayed out, not open for choosing.
The options on the PDF have values assigned to them, but they have never been used for automated reports and thus haven't been proofed, so you'd need to go on the a-i value of each choice in constructing the form, not the back-end assigned values.
I want to be able to edit the phrasing of the questions and the number of high/lows allowed.
I'm sure there's more you'd need to know, but let's start there.
