Wordpress PHP development

I'm looking for an experienced and multi skilled developer to help with with some PHP coding on a Wordpress site.
Amongst other things, the site captures data using the Ninja Forms plugin and I need some code to allow me to write the captured data to a Microsoft SQL database hosted in Azure when the form is submitted.
We already have a form which does this thatwas designed and implemented by another developer, so you should be able to reuse and adapt the existing code rather than having to create it from scratch.
The forms are to capture company appraisals and 121 meetings with our staff and we also want the users to be able to view forms that they have completed previously and view the forms completed for members of staff who work in their teams
