We need a form built for our customers

We need a form built for our customers - it will be conditional - please see journey map attached
1) you can access it on mobile and desktop
2) there will be a lot of pictures on there so it has to be clear how the information comes back to us ....



Basically form consists is this : but people need to add multiple snags... so add another snag , it needs to be very neat and professional
