Small php and css edits to a 'request quote' form

Hi there, I have a request quote form where a user adds in their details + uploads a photograph. The 'thank-you' page gets delivered to me and to the potential customer.

I need someone to:

1) Style the form so that when it arrives in the potential customer inbox, it has the same branding colours as existing form
2) Edit the form so that "name" and "email" are required fields
3) At the moment when it arrives in the inbox that photo won't display properly - please fix this so photo will show
