Search form to pass value to another form

Hello,

I need a simple form to get distance value from google maps and pass the value to my form.

I have found the code online here:

On the above link demo the "Get route" demo displays the map and driving directions.

I only need to get the value of distance in km and pass the value to my form.

I can pre populate the distance value by url :

Conclusion :

A form with two text fields
Location A :
Location B :

SUBMIT Button

When click submit it will calculate the distance and pass the value to url parameter to my form
