We are offering clients a simple 15 question risk survey questionnaire, which will be hosted on our website.

We need this questionnaire to be coded in PHP so when they complete it, it will provide the user with an onscreen risk rating (which we can calculate for you), and then send the user a full PDF report explaining the detailed reasons for their risk rating, to their email address.

    Does it have to be designed with more than just a basic form?

    Does it have to be in PHP? If you're happy for a different tech stack, I can roll this out for you in a couple days.

    our website is wordpress where it'll be hosted. We do have a react.js developer in our team if needed, but he feels this should be coded in PHP
