SignUp form that contains Username and Email
When submitted, user is emailed a random password (that is hashed and also stored in the DB)
Username (and email) is validated before submit and options redirect to login, or password reset
OR
User is sent a OTP or OTL which then logs them in to select a password of their choice.

DB is MySQL
Programatically correct with PDO / SQL Injection / Hashing /
HTML 5/AJAX preferred

Hello Mark,
As you mention "HTML 5" I suppose you want a design as well. Do you have another site to "copy" or an image as a reference?
