Project Brief
We have an existing system in place which works off paperbased forms (Please see attachment AS IS) and we would like to convert this form to a digital web form with backend database (Please see attachment TO BE)
The key requirements are all outlined inside of the form where we would like to gather data from a web based system which will be internally accessible on our intranet. The new form will have a selection of free text, dropdowns and mandatory fields. All of these must be created exactly as they appear on the template form. The form needs another field added in 'Email Address' for notifications and approvals to take place. Please note the form DOES NOT have to appear in the format supplied, just as long as all of the fields are transferred across.
The second requirement is to have a checkbox on the system which will take you to another form (Please see Isotopic Form) if this checkbox is selected, it will go to this form which needs to be filled out and emailed for approval - once the approver has selected 'approved' the form will be finalised and sent out to the appropriate department.
The process for this will appear as follows
- First Form Filled in with checkbox ticked
- Links to second form which needs to be filled in and is emailed for approval
- Is sent to Person XYZ with a WebLink for approval
- Person XYZ approves and the original person recieves notification that their form has been approved
- Form 1 and Form 2 both need to be submitted to Person 2
- Process complete
The system can be developed in any format you would prefer but it needs to have authentication built into the first layer to create the forms, and it will require authentication for the reporting aspect which is detailed below. Please respond to my post with "Chemform" so I have understood that you have read my entire brief.
Reporting will need to be done based on the fields in the form. We have a key requirement to know how many of each category are being logged, searchable records based on user, type, etc - all of the fields in the new form.
The system will need to run on Ubuntu Linux.
