Addition to modification of existing PHP/MySQL project

No generic proposals or form letters. Please mention specifics related to the overview below in your proposal.

Time sensitive--- needs to be done ASAP. Part of this project is complete as there is an existing site using the same central database. Need to create new applicable tables in db and write PHP for new functionality.

OVERVIEW -------------------
Programmer will need to take on/edit/amend existing codebase and move the project into GitHub.

Dealers login to submit auto warranty claims for their customers through a form.
- Some NEW form field options need to have autofill function from database tables.
- ADD ability for dealers to view past claims

Claim currently goes into database, email is sent to client
- ADD confirmation email to dealer with claim number

Admin area currently allows client to add or update dealers and reprint claims
- ADD field to specify which site this dealer belongs to
- ADD better filtering abilities for browsing/searching claims
- ADD ability to send an email to the dealer confirming payment/credit after the claim has been approved. Possibly form letter with claim customer and number pulled in.
- ADD csv file dump options with filters

Attached screenshots show file structure and db table structure. We will need to do a screenshare session to go through everything before work begins.

Budget is just an estimate. Will pay more for expedited work.
This is possibly an ongoing project. We need an efficient and reliable programmer!
