2 x large Jotform project

I am in urgent need of 2 long forms to be setup for me. function is more important then design, but it must still look professional as its for one on my consultancy clients.

I will provide a pptx of each form that clearing shows all fields needed, and what type of question these are>

form 1
Titled - C-Level Website Privacy Report
9 sections, about 40 questions
mix of short text, check box, radio, doc upload (xls, word, pdf) type input.
*all upload fields should be multi upload

form 2
titled - Website Privacy B-Level Report
5 sections, about 30 questions in total
mix of short text, check box, radio, doc upload (xls, word, pdf) type input.
*all upload fields should be multi upload

This forms are needed to be used on Monday (10am UK time)

I am not sure how you create forms to then add to my account so I would need your assistance. This work is protected by an NDA.
