Table App and Web App

I am looking for a table app to be built.
The app must also be able to be built into my website.

The App:
The app is a calculator that allows salon clients to build a tailored package for a salon they would like to go to and then pay for the membership through the app.

The way it works:
Client chooses the salon they would like to visit for 12 months.
Then they choose the services they would like to have - hair cut/ Colour/ treatment, etc and then the level of stylist per service.
from here they choose how often they would like to visit the salon over 12 months for each service.
They then pick the bonus services they would like.
The app will calculate a monthly payment plan for the client.
The the client signs up through the app.
