Simple edit of php image upload form to include upload to ID

Hi,

I am using a form which is attached below to upload images to my server.

The form uploads an image and stores the image in a column called id with a numeric ID value.

I need the form modified so that there is an input field on the image upload form which allows me to specifically select the numeric ID value for which i wish the image to be uploaded to.

The ID table column already exists on my database and i need to be able to select the specific ID column where i want the image to be stored under.

Example: I have an id column with id number 234 and the form should have an input field where i can type the number 234 so that when the image is uploaded it is uploaded to the correct database entry which contains id 234.

This should be a very simple job and take no more than 30 minutes for an experienced php programmer.

Work needs to be completed within the next 24 hours.
