Build Online Database

General information for the business: Store Management reporting database
Database management system (DBMS): Microsoft SQL Server, asp.net what suits YOU!
Description of requirements/functionality: I require a online database which we allow our offsite contractors to login and populate information into the database.

Requirements..
1: Login screen (user control access)
2: Ability to add photos into the database
3: forms for data inputting (exact number of forms to be confirmed but no more than 10)
4: formatted reports from one of the forms ( what is shown on page is to be in a printable format)
5: Email notifications (can be discussed)

Open to suggestions on design does not have to be fancy.

I am in the process of laying out the information needed for each form.

I will provide the domain name but can discuss hosting/support options.
