Restructuring of Data from one Excel to another

Hi, I need to extract information from one excel and re-formatted into another. I have attached the ORIGINAL spreadsheet and also an example spreadsheet which contains the right format. I have also started the example spreadsheet with a bit of data input so that the format is clear and concise.
