Formatting data to bring number values

What I need to do is write a formula for multiple criteria.
The raw data is in the DATA tab that houses all of the information that is being imported for an exported sheet into the DATA tab in column A thru L. On the data table I added columns M thru R to be able to tell me information like is the intervention true or false, does the time of day meet the night/weekend criteria, this may not be needed.
Column R is where I would like the score to be displayed based on the criteria being met.

Ultimately what I need to do is score the following criteria: on the Data Tables I have what the criteria is and the score it shall receive
1. if Column J (Intervention Text) is True then Column I (Mitigation) must have text in it and Column G (Condition) must be safe for a score of 10 pts.
2. Column J (Intervention Text) is True and Column I (Mitigation) is blank for a score of 0 pts.
3. Column G (Condition) is at risk for a score of 5 pts ; no matter what the other cells have in it. The at risk condition gives it a score of 5.
