Wordpress webdesign assistance

We need some assistance with creating a new header and footer for our current wordpress site to add some additional functionality to include:
- Search bar
- Sticky menu tab
- Sticky footer call to action button
Domain is www.rp-x.com, with 3 sub brands, www.rp-x.com/fitness, www.rp-x.com/health, www.rp-x.com/wellness
We have designer that is doing our layout and updates with in site, we need someone to create the changes needed to header and footer of site.
We also need some assistance with setting up forms to link to our CRM system, Dynamics 365/ Click dimensions if possible.
