Num. of web pages/modules: 2-3
Description of every page/module: I am looking for someone to help me set up a very simple site which generates an instant quote for potential customers which will let them sign up instantly after receiving it. Ideally it would be the same as this:

Almost all of my customers will be on mobile. I've not decided if wordpress or leadpages would be best.
Can you help?
Description of requirements/features: Ideally this would be made with wordpress/ leadpages. I'd be happy if this was done via an extra plugin.
