Photoshop doc converting to 1 wordpress holding page

I need a photoshop doc, converting to a Wordpress holding page please. Background is full width video. Bluehost hosting already has Wordpress loaded with domain. Rest of page layered on top of video consists of a logo, couple of lines of text, simple contact form (name & email to fill in). Would like a theme loaded that has a parallax effect that I can add to.
This is first stage obviously with potential to develop the rest of the site.
Need this first page really quickly.
