I will provide access to a staged copy of the website and screen caps showing the relevant MySQL table structure. Form is built in RSForm!Pro extension wherein query is implemented via admin GUI (see attachment). NDA required.

    The live site is MBUSI.COM and requires a login (Team Zone) to access/see the form.

    The staged site is intended to provide admin access after hire. You will have access (or I will provide) whatever you need, i.e., db table/cell records, super user access.

    I'm happy to provide answer(s) to any specific question(s) you might have.
