Gravity Forms Javascript Issue

I have the following issue where my gravity form is not displaying. It has a css class of display none which cannot be removed. The display none css is automatically added by gravity forms plugin when there is a javascript conflict. I am not sure what the conflict is but I believe it is from the theme as i have tried to deactive plugins and it still happens. The form is supposed to show here but as you can see it is blank.
