Need some advice on a questionnaire for my masters

I have designed an initial questionnaire (please see attached) for my masters project. The theme of which is how to maximise engagement with digital change projects in the NHS.

I need someone skilled in analysis (Probably SPSS) to tell me if what I have so far would be possible to analysis to masters level with reasonable results and then to recommend additional questions to strengthen the potential analysis that could be conducted.

Please provide some understanding/explanation of how you you think this can be achieved.

I am aware the questionnaire is potentially a little light and welcome suggestions for improvement.
