Roller banner design template

We are looking for someone to design a template banner for us, we will need to be able to edit the QR code and branding on the template.

The concept - We have launched an affiliate app for gyms and their customers. The gym members have to sign up to the affiliate via a QR code, we will send these banners to each gym that signs up to our affiliate scheme. The reason we need to be able to easily change the QR code and logo.

The site offers discounted fitness accessories, prizes and reward points.

The products include - nutrition, apparel, accessories such as belts, wraps, grips and speed ropes.

We want the banner to have a simple heading "The Armoury" the QR code and a "Join Now" text - with a message along the lines of "sign up today and receive £5.00 reward points"

We also want the banner to include the member perks as listed above. Show examples of some products and titles of products categories they can expect to find.
