Simple brand guideline document.

I have just established an executive coaching business and have a newly designed logo, colour palette, fonts, and some campaign graphics which I need packaging into a very simple brand guideline document. All the assets are existing, but I need it to be in a presentable format for briefing on upcoming work. I know there are some off the shelf templates that exist so I would be happy for someone to use one of these if it is includes all the relevant guidelines to be able to adequately brief subsequent work (e.g. it does not need to be an original template if that is not necessary).
