Web Designer Wanted

I am looking for a simple website to be built which doesn't need to do much but graphically needs to look very creative.

It's essentially a three page website with a forum with a few drop down fields for people to choose from and write a comment on. These comments are all searchable and viewable by the field parameters eg date, time, place etc. Need to be able to monitor and delete posts if they're inappropriate

Needs to be SEO optimised as well and have space for advertising

Domain and hosting will be supplied.

Please could you let me know:

1. When you can start.
2. What software you propose using.
3. Examples of other websites you have created before.
4. Total cost to complete job.
